Job Summary and Qualifications
What you will do in this role:
- You will be responsible and accountable for coordination of services through an interdisciplinary process that provides a clinical, financial, and psychosocial approach through the continuum of care.
- You will balance individually identified patient and family needs with the efficacious and cost effective utilization of resources.
- You will share responsibility for the evaluation, planning, implementation, coordination and supervision of Case Management Department operations, functions and staff in alignment with the annual UM/CM Program as well as applicable regulations and standards.
- You will be responsible for supervising day-to-day operations, including the staff schedule, assignments and caseloads to ensure appropriate hospital-wide coverage.
- You will provide expertise and staff support for problem-solving complex cases and issues and consults with the Director/other resources as needed.
- You will collaborate with Director and participate in staff evaluation and performance activities including monitoring, coaching, counseling, educating and developing action plans as needed.
- You will represent CM and attend meetings for assigned committees, workgroups and teams.
- You will participate in required CM department data collection, medical record reviews/audits, reporting and analysis.
- You will develop and supervise new staff orientation plans and the department's annual education plan, including staff core competencies.
- You will share responsibility with the CM Director for some "24/7" management accountability and provides coverage for the Director's time off as needed.
- You will demonstrate an understanding of the CM department's budget and practices cost-effective resource utilization.
- You will participate in CM Department quality, safety and risk management activities CQI program.
What qualifications you will need:
- Completion of RN Diploma or related health care program from accredited program, college or university.
- Must possess current Florida State RN licensure.
- Minimum of three years recent acute clinical practice or related health care experience.
- Excellent written and verbal communication skills; strong interpersonal skills; ability to collaborate effectively.
- Ability to prioritize work assignments, multi-task and manage time efficiently.
- Excellent problem solving skills.
- Proficiency in basic PC skills; Excel, Microsoft Office.
- Strong knowledge of governmental and private organization and community resources.
