Aviata health Group is looking for a Regional Director of Social Services - Skilled Nursing/ Long Term Care

Job Summary:

Aviata Health Group is seeking a highly qualified and experienced Regional Director of Social Services to join our team! As a key member of our leadership team, you will be responsible for providing exceptional social services to our residents and their families. You will also assist in building a culture of excellence within our care centers by leading and managing the social services department.

Responsibilities:

• Provide coverage, stabilization, and progression of the social services department during times of vacancies and on-boarding of newly hired leaders
• Conduct assessments of social service department systems and processes to ensure compliance with company, state, and federal regulatory practices
• Consult with management staff to develop and implement policies and procedures for social services operations
• Conduct leadership evaluations of care center department heads and create plans to upgrade and develop talent
• Stay up-to-date on current federal and state regulations and make recommendations on policy changes
• Implement policies and procedures for identifying and addressing medically related social and emotional needs of residents
• Participate in community planning and discharge planning, and develop and implement social care plans
• Provide consultation to staff, community agencies, and families to solve resident needs and problems
• Review and develop plans of correction for social service deficiencies noted during survey inspections

Requirements:

• Master's Degree in Social Work (MSW) from an approved school of Social Work
• Minimum of one (1) year experience in long-term/skilled nursing care
• Must possess knowledge of accreditation standards and compliance requirements
• Must be a member in good standing in the National Association of Social Workers
• Must be willing to travel throughout Central Florida
• Must live in Florida
• Demonstrated ability to assess resident needs, develop and implement a plan of care, and coordinate discharge planning process
