Responsibilities:
- Develop and implement individualized plans of treatment for each patient based on initial and ongoing evaluations, including short- and long-term goals
- Implement treatment plans on a consistent schedule and adjust plans according to residents' progress
- Document residents' progress according to approved medical documentation and facility documentation procedures
- Evaluate residents' mobility, strength, and current level of function, setting goals to improve these areas
- Report resident progress to residents, caregivers, loved ones, facility staff, and healthcare professionals involved in ongoing treatment/care
- Assist other facility staff members as needed to provide care and create a clean, orderly, and effective environment for rehabilitation and physical therapy
Qualifications:
- Graduate of School of Physical Therapy
- Minimum of three months recent experience as a Physical Therapist working in a nursing home environment preferred
- State of Florida Physical Therapy License
- CPR Certification
