BayCare Health Systems is currently seeking a Discharge Planner Pool to join our outstanding and compassionate team at Northside Behavioral Health!

• Facility: Northside Behavioral Health
• Location: Tampa, FL
• Status: Pool, Hourly
• Shift Hours: 8:00 am-4:30 am
• Weekend Work: 4 Weekend Days/ Month
• On Call: No

The Discharge Planner Pool responsibilities include:

• Provides assessment, linkage, monitoring, planning and advocacy for adult and or children experiencing acute mental health and or substance abuse issues in an acute care inpatient setting.
• Possesses a clear understanding of discharge and transition planning and linkage with community resources.
• Oversees legal status issues related to the individuals involuntary or in accordance with the Baker Act regulations.
• Provides support as active member of the treatment team.
• Will perform other duties as assigned by supervisor.

Travel may be required.

Required Certification:

• BLS (Basic Life Support)

Required Experience:

• Behavioral Health - 1 year
• Child and Adolescent - 2 years (SIPP Only)
• Acute Care - Preferred

Required Education:

• Bachelors - Counseling/Health Education/Related Field/Social Work/Human Services/Psychology/Nursing
• Master's in human services - Preferred
