The Advanced Practice Provider (APP) provides comprehensive nephrology and kidney transplant care to individuals, families and groups in the inpatient setting. Responsibilities include the diagnosis and management of acute and chronic kidney disease, management of kidney transplant recipients, and coordination of care for complex renal conditions. The APP will emphasize health promotion, disease preventions, dialysis management, and transplant aftercare. This role provides the opportunity to practice autonomously and collaboratively within an interdisciplinary team of physicians, surgeons, APPs, nurses, and other specialist. All TGH team members are responsible for performing job duties in alignment with the mission, vision, and values of TGH.

Responsibilities
- Evaluate, diagnose, and manage acute and chronic kidney disease, electrolyte disturbances, and glomerular disease in the inpatient setting
- Provide post-transplant care for kidney transplant patients, including immunosuppressive medication management, rejection surveillance and management, infection monitoring and treatment, and coordination of care with nephrologist and transplant surgeons
- Manage dialysis-related care, including hemodialysis, peritoneal dialysis and continuous renal replacement therapy in ICU settings
- Perform comprehensive assessments, order and interpret diagnostic and laboratory testing, and develop evidence-based management plans
- Provide care for nephrology patients int he ICU, including dialysis
- Collaborate with the interdisciplinary team to optimize outcomes and ensure continuity of care
- Participate in nephrology consult services including transplant recipients, complex renal cases, and critically ill patients
- Works autonomously as well as in collaboration with a variety of individuals to diagnose and manage clients' health care problems
Qualifications
Education
- Master's degree Nurse Practitioner or Physician Assistant
License - Issued by Florida or Compact State
- Advanced Practice Registered Nurse or Physician Assistant
Certification
- Acute Care certification, Adult Gerontology Acute Care certification or Nationally Accredited Physician Assistant Board certification
Work Experience
- Experience as Advanced Practice Registered Nurse or Physician Assistant preferred
